What Does “LGBTQ+ Affirming Therapy” Actually Mean?
You may see on a therapist’s website or on therapy directories that they offer “LGBTQ+ Affirming Therapy.” But what does that actually mean? The word affirming can sound vague and is often misunderstood. Not all therapists who work with people in the LGBTQ+ community provide truly affirming care.
In this post, we’ll break down what LGBTQ+ affirming therapy really means, how it differs from “LGBTQ+ friendly” care, and what language to look for in therapist profiles if you’re seeking inclusive counseling or LGBTQ+ therapy in Michigan—including right here in Jackson, MI.
The Difference Between “LGBTQ+ Friendly” and “LGBTQ+ Affirming”
Do “LGBTQ+ Friendly” and “LGBTQ+ Affirming” Mean the Same Thing? From my perspective, there are some important differences between these two terms.
LGBTQ+ friendly often means that a therapist is welcoming to people in the LGBTQ+ community.
It’s a good start—but being friendly doesn’t necessarily mean the therapist has training or experience in LGBTQ+ mental health.
LGBTQ+ affirming, however, goes a step further.
An affirming clinician actively validates and advocates for the needs of LGBTQ+ individuals. They’ve completed additional training or sought education to understand the unique challenges that LGBTQ+ clients may face. Therapists who provide LGBTQ+ affirming therapy are intentional about creating safe, inclusive spaces for clients to explore their sexuality, gender identity, and relationships.
In short: LGBTQ+ friendly means everyone is welcome. LGBTQ+ affirming means the therapist intentionally supports, understands, and empowers people in the LGBTQ+ community through informed and inclusive counseling.
What Affirming Therapy Looks Like in Practice
What should you look for when searching for a therapist who truly provides LGBTQ+ affirming therapy? There are a few key signs that a clinician has taken time to learn, reflect, and intentionally create an affirming therapy experience for people in the LGBTQ+ community.
Below are some indicators that your therapist is providing an affirming, inclusive, and supportive space:
They use your pronouns and name correctly. They use your correct name and pronouns throughout treatment. While your legal name may appear in documentation, your therapist honors the identity you share with them.
Their paperwork and forms use inclusive language. Intake forms or questionnaires reflect inclusive options when referring to gender, sexuality, or relationships. They acknowledge different partnerships, pronouns, and identities.
They don’t make assumptions about your sexuality or gender. Affirming therapists understand that identity can’t be assumed or labeled based on appearance.
They collaborate on your therapy goals. They don’t make assumptions about what you “should” work on because of your identity. Instead, they invite you to set goals together that align with your values and needs.
They’re comfortable discussing intersectionality and identity. An affirming therapist recognizes how factors like race, culture, gender, and sexuality intersect to shape your experiences and mental health.
These are just a few examples of what LGBTQ+ affirming care can look like in therapy. Every therapist’s approach is unique, but intentionality, respect, and inclusivity should always be at the core of your experience.
Why Affirming Care Improves Mental Health Outcomes
Gender-affirming care in particular, has been shown to improve mental health outcomes. It can reduce depression, anxiety, and suicidality. Approaching therapy from an affirming stance creates safety and trust. These are essential for doing deeper work and making progress toward personal goals.
Affirming care helps people feel seen, heard, and respected. It prevents clients from feeling dismissed or overlooked. Providing inclusive and validating therapy is the standard, not a specialty.
Questions to Ask When Searching for an Affirming Therapist
If you are looking for a therapist and wondering if they provide LGBTQ+ affirming therapy, here are some questions to ask during your consultation call:
What experience or training do you have working with clients in the LGBTQ+ community?
How do you handle pronouns and gender identity in your practice?
Do you provide or coordinate gender-affirming medical care?
What is your approach to providing affirming therapy for those in the LGBTQ+ community?
If something feels off when asking these questions, trust your gut. Keep searching for a therapist who makes you feel safe and provides the affirming care you deserve.
Red Flags to Watch Out For
Unfortunately, some providers claim to offer LGBTQ+ affirming therapy but do not fully follow through. They might advertise it for marketing or feel it is something they “should do.” It is okay to look closer. See if their actions match their words.
Here are some red flags to watch for:
Performative inclusivity: A rainbow logo may be on their website, but they have little experience or training in affirming therapy for clients in the LGBTQ+ community.
Minimizing identity: They say they “treat everyone the same” regardless of identity.
Limited awareness: They are unfamiliar with current LGBTQ+ terminology or national issues.
Avoiding topics: They avoid conversations about sexuality or gender.
These signs do not automatically make someone a bad clinician. However, they suggest the therapist may not be as skilled in LGBTQ+ affirming therapy as they claim.
